This program would work to serve our patrons that do not have the ability to come to the library to check out books and magazines. At this time we can only offer this service to the residents of the City of Allegan.

Library Patron Eligibility:
Once a library patron finds himself/herself in a position that they are not able to come to the library to check out materials or they have difficulty getting in and around the library, they would be eligible for this program.

Ways a patron could qualify for this program:

They are homebound for medical reasons. Illness, incapacity, etc.
They are homebound due to taking care of family member

**A medical reason could be short or long term and this program can be turned off at any time once started. **
**There may be a possible addition of audio and video materials at a later date**

A patron would be eligible for this program if they could occasionally get to the Library or they have a friend or family member that could pick up/drop off materials for them. Also, a patron must have a working phone number and home address. No PO boxes will be accepted.

Starting the Program:
A simple application would be required to be filled out. This application can be mailed, emailed, or
filled out in the library.
Once the application has been verified and approved, the patron will be enrolled.

Responsibliities of Enrollment :

No more than 5 materials will be selected at a time.
Materials will be held, mailed, or picked up by the patron or designee.
The patron may keep the materials for 5 weeks.
Late fees will be waved for patrons in this program.

**Library staff will check materials for no repeat check outs.**

For the Library:
A member of the library staff would complete the application and put into the system that the patron is in
the Services Program. If the patron does not have a library card, one would be issued at this time.

A card can be registered 3 ways:
- in person
- through the mail
- through a designee.


The patron would be linked to a staff member or volunteer in the library that would:
Show in the system as that patrons representative.
Be the in Library contact for the patron.
Choose materials for that patron.
Set up with the patron the way that the they would like to receive their material:
- pick up at front desk
- have somebody else pick up the material for them.
- have the materials mailed to them through the USPS.

Pack materials with return information and due date.
Check materials back into the system for the patron.
